New Patient Information
What to Expect During Your First Visit
On your initial visit, you will be asked to complete a health history and information form. (Forms are available below if you would like to complete them ahead of time and bring to your first visit.) Please bring:
- All current health insurance and prescription insurance cards
- Your driver’s license
- Previous health records
- A list of all medications you are currently taking including the strength and how often you take them
- A list of all your doctors and their telephone numbers
- Your completed Living Will and Medical Power of Attorney, if you have these
Be prepared to pay a co-payment and/or a deductible, if needed. All information given to us is strictly confidential and will not be released to anyone without your approval.
First Questions
We understand that you will have questions about your diagnosis and treatment options, and we want to give you as many answers as possible. During your initial office visit, you and your doctor will discuss your diagnosis in detail. You will learn about treatment options, what is involved and how Hematology/Oncology Clinic will support you through the process.
